Coaching Certifications: Please provide your personal NCCP #, and list all certifications (with cert. numbers) that you have completed

Coaching References: Provide three references that we can contact in regard to your coaching background and experience. One coach you have coached with, one coach you coached against, and one parent. Please provide names and contact information (phone & email address)

In the last five years as a coach, manager, or parent, have you ever been removed or ejected from a youth sports game or activity by the umpires or officials? If yes, please provide details of the ciircumstances.

Explain your reason(s) for interest in coaching All-Star baseball/softball.

Explain your definition of a successful baseball/softball season.

Explain your approach to communicating with parents and handling issues that may arise.

Explain your approach to utilizing assistant coaches.

Explain how you would handle a disagreement on a call made by an official.

Explain your approach to developing players (developing skills, playing time, discipline, pitcher & bullpen management, etc.)

Explain the emphasis that you place on winning.

Provide Practice Plans showing how various practices will be run to develop players in all aspects of the game and what the coaching staff will do for each of: 1.5 hr practice, 2 hr practice, 1hr BP Cage Practice
